Lost/Stolen/Broken Staff Devices

Lost or Stolen Devices

  1. Staff should alert their Supervisor and file a report with local police.
  2. Submit a CSR with the Technology Department.
    • Provide a copy of the police report and/or the case number.
    • Provide as much information about the device as you are able.
  3. If a staff person needs a new device, please follow instructions under: STAFF DEVICES:

Broken Devices

  1.  Submit a CSR with the Technology Department.
    • Provide as much information about the device's condition as you are able.
    • Provide the serial number, if you can locate.
    • Provide the asset tag number if you can locate.
