Bridge Team

The Need
Michigan's children and adolescents are facing increasing mental health needs and exposure to trauma. Schools and community partners are collaborating to respond more effectively, coordinate care more efficiently, and support the whole child and family.
The Impact
Mental health has a powerful impact on learning and well-being. Conditions such as anxiety, depression, and trauma can lead to attendance challenges, academic difficulties, and increased dropout risk. With the proper supports--and a coordinated plan--students and families can thrive.
Why Schools?
Students spend most of their time in school, making schools an ideal place to identify warning signs, build resilience, and connect youth and families to support. As learning environments, schools also create opportunities to teach emotional regulation, self-awareness, and help-seeking skills in safe, inclusive spaces.
How can we help
Bridge Clinical Team
- Individual and group counseling for students
- Liaison support connecting schools, families, and community agencies
- Care coordination and case management
- Consultation and coaching to school staff on evidence-based strategies
- Support for multi-tiered systems of support (MTSS) and positive behavior frameworks
Crisis Mental Health Consultation
- Short-term coordination during and after crises
- Return-to-learn and reintegration planning following hospitalization or justice involvement
- Safety planning and linkage to ongoing community-based care
- Collaboration with families, schools, and providers to stabilize and reconnect students
Bridge Wraparound Program
What It Is
High-Fidelity Wraparound is a structured, family-driven strengths-based team process for youth with complex needs. It "wraps" formal and natural supports around the family's own goals, ensuring that voice, choice, and culture guide every step.
What We Do
- Facilitation: Bridge Wraparound Facilitators guide youth and family teams through engagement, planning, implementation, and transition.
- Family & Youth Voice: Plans reflect each family's culture, strengths, and priorities.
- Team-Based Coordination: Facilitators align school, community, and provider efforts to reduce fragmentation.
- Safety & Crisis Planning: Teams develop individualized safety plans that work across home, school, and community settings.
Who It's For
Youth and families navigating multiple systems (such as schools, community mental health, juvenile justice, or child welfare) who need a coordinated plan to align supports.
